Eliot Institute Sponsor Form

This form must be completed for any camper under 18 years of age who attends an Eliot Institute conference without the camper's immediate family, parent or legal guardian. It must be submitted at the same time as the:

1. Application of the sponsoring family;
2. Eliot Institute Medical History & Treatment Authorization; and
3. Necessary deposit.

An adult sponsor must be at least 25 years of age and may sponsor only one camper per conference. Please note you must complete all three parts of this form - one for Parent/Legal Guardian, one for the sponsored child, one for the sponsor.

This sponsored camper has my full permission to attend an Eliot Institute conference at( ) Seabeck, WA  ( ) Naramata, B.C. from ___________ to ____________, 200____ and to participate fully in all Eliot Institute activities, both on and off site.

I authorize (please print) __________________________________ to be the adult sponsor of my child during the Eliot Conference and en route to and from. I agree that:

1. If my child cannot travel to and from this conference with the adult sponsor, I take responsibility for providing transportation, which I will fully explain to the adult sponsor. I understand that my child may not arrive at camp before, nor leave camp after the adult sponsor. I understand that if the sponsor must leave camp prematurely, my child must leave as well, or I must pick up my child before the sponsor leaves.
2. I will ensure that my child receives from the adult sponsor copies of all information from the Registrar and Dean sent before the conference.
3. I will send to the adult sponsor the fees for my child in time for the adult sponsor to send them to the Eliot Registrar before the payment deadline.
4. I will discuss with my child his or her responsibility to follow the rules and expectations of Eliot Institute, of staff members, and of the adult sponsor.
5. I authorize the adult sponsor to exercise all powers given to Eliot Institute under the Eliot Institute Medical History and Treatment Authorization, which I enclose.

1. Leave camp when my sponsor does, and not arrive at camp before my sponsor does.
2. Make certain that I obtain and read all mailings that come to my sponsoring family;
3. Make certain that my payments are sent to my adult sponsor in time for the sponsor to send them to the Eliot Registrar before the payment deadline; and
4. Act as a reasonable, prudent and responsible camper in regard to my adult sponsor.

I, (please print) ______________________________________, am sponsoring a camper under age 18. I agree to:

1. Include the sponsored camper's name on my application;
2. Include this fully completed sponsor form, and the required deposit for the sponsored camper, with my application;
3. Have the sponsored camper housed with my family;
4. Make sure that all correspondence from Eliot Institute regarding the conference we are attending is forwarded to the sponsored camper;
5. Assume responsibility for the welfare and conduct of the sponsored camper during the Eliot Institute conference, and en route to and from camp;
6. Establish and monitor a reasonable evening curfew for the sponsored camper;
7. Fulfill my duties as a reasonable, prudent and responsible parent would;
8. Arrive a camp with or before the sponsored camper, leave with or after the sponsored camper; and
9. Ensure my sponsored camper's return to parents should it become necessary for me or my sponsored camper to leave the conference before its completion. If necessary, I agree to accompany the camper myself.
