Application Form: To attend an Eliot camp/conference or to be put on the waiting list, fill out on-line, then print out and mail. Instructions and rules are included and/or linked to the application. All staff and speakers must complete the application form. However, speakers and spring workshop leaders need not send a deposit unless they are bringing other family members.

Financial Assistance Form: Read the instructions and criteria upon which financial assistance to attend Eliot camps is offered before completing a request for financial assistance.

Conference Volunteer/Suggestion Form: Suggest speakers or apply to be on conference staff. Some positions earn camperships; a few receive small stipends.

Adult program evaluation form: If you forgot to turn in an evaluation form at the end of the conference you attended, you may do so by filling out this form and sending to the registrar.  Also available are the youth program evaluation and the children's program evaluation.
